This article outlines how Admins and Loan officers can view and edit the Lead Gen Form. The Lead Gen Form can be customized at the Company, Region, Branch and LO level. The Lead Gen Form is currently only available on Web.

Company, Region, Branch Lead Gen Form: Admins can view the Lead Gen form at the Company, Region or Branch level by going to the Custom Forms tile on the Company Settings, Region Settings, or Branch Settings page, and then click Lead gen form tile.
Loan officer Lead Gen Form: Admins and Loan officers (LOs) can view the Loan officer Lead Gen Form under Settings > Lead Gen Form.You can also can view the Loan officer Lead Gen Form on the Share App page or when sharing you can view and edit the current Lead Gen Form by clicking the 'View or edit form' button.
- The Lead Gen Form page will show all available questions, and which are currently enabled. This will also show if the region/branch/LO is inheriting from the Company level or has their own customization. The Lead Gen Form has two sections: Home Purchase and Home Refinance.
- You can preview the entire Lead Gen Form or individual questions. To preview the entire form, click 'Preview form' shown at the top right of the page. To preview a single question, click 'Preview' to the right of the question you would like to preview.
When done previewing, simply click the 'X' in the top right corner of the preview banner on the page. This will take you back to the Lead Gen Form page. - To enable or disable a specific question on the Lead Gen Form, click the toggle next to the question under 'Show in form?'.
If there is a lock symbol next to a question, that means that this question cannot be disabled. - You can customize the Lead Gen Form by toggling a question on or off, which will cause this pop-up to appear. Select 'Continue' to customize.
- In order to remove customization or set the Region/Branch/User to the default Lead Gen Form click the three dots next to the Preview form button and select Restore Defaults. This will reset the Region/Branch/user to the defaults of the next highest level of default settings. For example, if you restore the defaults on an LO account, it would inherit the defaults of the customization at the Branch level if enabled. If there is no other customization at the Branch or Region level, it would restore the defaults set up at the Company level.
